Use this family survey to invite parents and caregivers to share information that helps teachers plan activities and communicate with each child’s family. The printable form includes spaces for the child’s and caregiver’s names, home languages, interests, strengths, and newly developing skills. Families can leave questions blank and use the final prompt to request a conversation, supporting welcoming, two-way communication. Reach for it during family intake, conferences, or program planning when you want to build on what caregivers know about their child.
